This web part can generate form based data entry from any SharePoint List.

Most SharePoint lists and libraries include data entry forms when you want to add an item, but what happens when you want to allow a certain group
of users to add items to a list, but not be able to view the items? or when you need to embed your form in your page? The
SharePoint Form Generator
allow all users to add entries to the list, and can be used to generate rich UI forms with field validation to be embed in your pages. The Web Part supports all types of columns used in SharePoint lists.
